What info and/or forms do I need to get a state issued ID card? Do I need to bring my social security card?

I lost my social security card, and my driving permit is expired. I want to get a state issued ID, but I am not sure if I need to bring in my actually SS card, or if it is ok if I just know my SS#. I can't get a new card unless I have a valid ID so I am kind of stuck as to what I should do. HELP!
