had Permant Handicap placard, it expired Why do I need a Dr to fill out form just to renew my old one ?

I moved from one part of Penna to another part, in the process of moving & getting married my placard expired, I noticed that it expired, & sent in form to get it renewed, I had had it since 1986 it was a PERMANANT one. Why do I have to go to my new doctor to have him fill out a form for me for a new one ? Why can't they find a record of mine when I sent them a photocopy of my card, & the wallet card ? I have Asthma, panic attack, fibromylagia ( which causes me to stiffen up at times from pain), & Lymphadema. Is there anyone else that I can get to sign this form since my new doctor won't ? I think he won't sign it because I drive a motorcycle also, but I also know of people who also ride & have canes or some other health problem & still have a placard.
